Shouldn't the title of Monkey Island 2 be "Monkey Island: LeChuck's Revenge" (or "LeChuck's Revenge: Monkey Island")? The title screen doesn't actually show the number "2".

Ingame title doesn't always take precedence if the other is notable enough. In this case the "2" is everywhere else.

I'll try to explain this, it is a bit difficult / nuanced.
I think there is some confusion that has stemmed from discussions we've had over the years about prioritizing the in-game title. However, that is mainly done in certain cases of ambiguity, but not necessarily *always* the chosen title, when looking at all the facts and circumstances.
We continually discuss and evolve over the years, and we've had many discussions among staff about title rules and priorities, so I can't really speak to every single past decision and why it was made.
Titles are very difficult sometimes, in terms of determining which is the main one to be used for the database. We've tried a few different approaches over the years, including a generalized line regarding prioritizing between in-game -> disc -> cover, but that doesn't always work as a hard-line rule, as there are situations where that would result in not the best title being used.
It is very common for games to include logos and/or images on splash screens or title screens that in some cases do not fully represent the game's title, so we sometimes have to take a more advanced approach and consider other factors within the artifact, to determine the best title to use.
I realize this introduces some level of subjectivity into the process, but it is very difficult to apply a "strict" rule to titles, without compromising things and introducing less-than-ideal titles and inconsistencies.
The Main Title for redump/dat purposes, can sometimes be one of the most difficult and complicated elements of all the fields we document here at redump. We try to create rules that are consistent and easy to follow, but it can be very difficult sometimes, so we have a set of guiding principles that we follow but then sometimes review the item in question.
What we at least try to do in some cases, is include an alternate title, or additional information in comments, to help explain some of the discrepancies, in case other decisions are made over time.
We'll continue to review and iterate on this process, and I hope that over time we can introduce more title fields to better accommodate different titles across items - we just don't have that ready yet and it will take some work.
