Okada Tooru. Though it's mostly written "Tohru Okada" in practice (e.g. on Discogs).

Suraimu sounds almost like Fainaru Fantaji for me (instead of Final Fantasy), Slime is better.

Regarding the last line, 伊与部 功 = Takumi Iyobe? Is this properly romanized.
