There are 2x "CD Sonic the Hedgehog" beta dumps with, I believe, naming/region issues:

This entry is titled "CD Sonic the Hedgehog" although the title screen has already been altered to "Sonic the Hedgehog CD" like the final game..

Any input welcome!

The header still says CD-SONIC THE HEGEHOG and the disc label is written as CD-Sonic, so it's not obvious.

Yes... but something funky is going on.
On real hardware, unmodified JP VA4 + Megasd, it will only boot with US bios. Forcing JP bios loads it as an audio disc, with no option to launch CD-ROM/Program.
Similar behavior on emu (Kega Fusion), bios auto-selection boots it as US and forcing JP bios doesn't allow to launch it, like on real hardware..

Doesn't the title screen take precedence ? I would have thought so, but yeah not obvious when the rest says otherwise.
