New Dumper, Help With Error Log?

First time dumping, PC CD-Rom, using a Plextor 716A with an old USB adapter I already owned.

The dump completed but there are entries in the subError log I wanted to ask about before submitting.  Here is the log:

I tried different drive speeds still errors.

So I know how to proceed, could this mean I need a different IDE/USB adapter? Or could it mean the laser bad in my Plextor PX-716A? Or could it be both?

To my understanding, subchannel data doesn't have a complete checksum and can't be error-corrected in all cases, but it isn't directly entered into the database either so having some errors doesn't matter.  I.e. it's fine to submit even with some errors in the subchannel data, since the subchannel data isn't part of the final database entry.
